Job summary

La Familia is seeking a Clinical Case Manager to support re-entry efforts in Alameda County. You will provide intensive case management for adults with serious mental illness, linking them to housing, employment, and health resources while coordinating with clinicians and probation partners.

The role emphasizes trauma-informed care, engagement, and ongoing collaboration within a team dedicated to improving client outcomes and community reintegration.

Qualifications

• Bachelor’s degree in Social Sciences field and 2 years of mental health experience.
• Bilingual English/Spanish required for effective communication with clients and partners.
• Formerly incarcerated or justice-system-impacted experience highly preferred.

Responsibilities

• Provide intensive care coordination and case management for justice-involved adults with serious mental illness.
• Engage clients to promote treatment participation and connect to housing, employment, and health resources.
• Coordinate access to medical, dental, psychiatric services and public benefits while ensuring continuity of care.
• Maintain accurate documentation and participate in weekly multidisciplinary meetings.
