Qualifications

• Required bachelor’s degree in social work or related field.
• Degree in social work, public health, or mental health counseling with two years of experience, or a master’s with one year of experience.
• Advanced knowledge of professional practices and guidelines; ability to act per program procedures.
• Caseloads may flex based on need and acuity of the population.
• Bilingual Spanish speaker is required.

Responsibilities

• Monitor progress of each client through face-to-face, escort, written, electronic and telephonic outreach and planning activities.
• Complete client-centered assessments to identify medical, behavioral health, and social needs and goals.
• Develop and document daily appointment schedules and participate in supervision as scheduled.
• Develop and update person-centered care plans driven by assessments; share plans with client and Health Home partners.
• Maintain an accurate caseload and respond to changes including discharge or transfer.
• Collaborate with service providers and participate in case conferences to monitor health status and plan care.
• Review crises with supervisory staff and involved legal entities; provide services to meet care plan objectives.
