Job summary

Caring for People Services Inc is seeking an in-office Care Coordinator to assist the Care Manager with coordinating services for a client caseload and supporting intake, assessments, and discharge processes.

The role requires 2+ years in home care or related fields, leadership experience, and bilingual Spanish/English. Travel to clients’ homes and on-call rotation are included; hours are Monday–Friday, 8:00am–5:00pm with possible after-hours as needed.

Qualifications

• This position requires a high school diploma or equivalent; an associate degree is preferred.
• 2+ years in home care, healthcare, human services, or related field; supervisory or team-lead experience is a plus.
• Bilingual Spanish and English is required; ability to pass a background check is necessary.
• Must be willing to travel to clients' homes and work in-office hours.

Responsibilities

• Assist Care Manager with coordinating services for assigned client caseload.
• Assist in client intake, service initiation, and reassessments.
• Assist with client visits, documentation, and service observations.
• Communicate with clients, family members, healthcare providers, and care team.
• Provide direct care during emergencies or staffing gaps; participate in on-call rotation.
• Maintain accurate caregiver scheduling and service records; comply with policies and standards.
