Job summary

NYPCC-New York Psychotherapy Counseling Center is looking for a Bilingual Care Coordinator to join their team in New York. This full-time position involves managing a caseload of Health Home Care clients and assisting in developing Comprehensive Care Plans. Candidates must have a Bachelor's Degree, be bilingual in English and Spanish, and possess experience with RMA. NYPCC offers competitive salary between $47,000 and $60,000 annually, along with comprehensive benefits including medical, dental, vision and student loan pay down incentives.

Qualifications

• MUST be bilingual (English/Spanish).
• Bachelor's Degree required.
• Experience with RMA required.
• Experience with HARP clients preferred.
• Possess knowledge of community services.

Responsibilities

• Manage a caseload of Health Home Care clients.
• Assist in developing Comprehensive Care Plans.
• Address various service needs including housing and medical care.
• Work with the Care Team to provide client-centered services.

Skills
