NADAP, Inc. in New York, NY seeks a Bilingual Spanish Care Manager to support Medicaid eligible clients through assessment, care planning and service coordination. The role is based in Midtown Manhattan with a hybrid four or five day work week.

You will collaborate with medical and behavioral health providers, educate clients about diagnoses and treatments, and connect them to community resources. A Bachelor's or equivalent in human services is required, with Spanish proficiency.

Qualificações

• Bachelor’s Degree in human services, social work, or related field
• OR Associate’s degree with 1 year of case management experience
• OR High School Diploma with 2 years of case management experience
• Excellent organizational, communication and computer skills
• Bilingual Spanish speaker required

Responsabilidades

• Assessment and care planning for Medicaid eligible clients
• Coordinate services with medical and behavioral health providers
• Facilitate access to care and promote better health outcomes
• Educate clients on diagnoses, medications and treatments
• Link clients to community resources and entitlements
• Engage with clients in home, doctor visits, or community settings
