Job summary

NADAP is seeking an Intensive Care Coordinator to provide assessment, care planning, and service coordination for eligible clients in New York City.

The candidate will work closely with medical and behavioral health providers to stabilize participants and enhance health outcomes. Duties include monitoring client progress, developing care plans, and maintaining collaborative relationships with service providers. A Bachelor's Degree in Social Work and experience with severe mental illness are required.

Qualifications

• Minimum of three years of job-related experience providing care coordination services.
• Experience working with participants with severe mental illness required.
• Ability to travel within Manhattan, Queens, Brooklyn, and Bronx.

Responsibilities

• Provide assessment, care planning, and service coordination for clients.
• Monitor progress of clients and ensure they meet Care Plan objectives.
• Develop and update person-centered care plans in collaboration with clients.
