Job summary

NADAP Inc. is seeking an Intensive Care Coordinator to assess, plan, and coordinate care for clients with behavioral health, mental health, and medical needs. You will work with Health Home partners and providers to ensure comprehensive care and reduce costs.

The ICC will monitor progress, perform functional assessments, develop care plans, manage caseloads, and facilitate referrals to medical, behavioral health, and social services. Strong collaboration and documentation are essential.

Qualifications

• Bachelor’s Degree in Social Work, Human Services, or related field required.
• Degree in social work, public health, or mental health counseling preferred; two years of experience, or a master's with one year.
• Caseloads may flex based on need and acuity of the targeted population.

Responsibilities

• Monitor progress of each client through face-to-face, escort, written, electronic and telephonic outreach and planning activities.
• Complete client-centered comprehensive functional assessments of medical, behavioral health, and social needs/goals.
• Develop, adhere to and document daily appointment schedules; inform supervisor of scheduling changes.
• Develop, review, and update written/electronic person-centered care plans driven by assessments; share with client and Health Home partners.
• Maintain an accurate caseload and respond to changes including discharge or transfer activities.
• Collaborate with service providers to monitor care level and health status; participate in case conferences as needed.
• Address crisis situations promptly with supervisory staff and involved entities.
• Provide services to meet Care Plan objectives, assist with entitlements, housing, transportation, and referrals.
• Utilize EHR and NADAP database tools to maintain documentation and treatment records.
• Performs other duties as assigned.

Education
