Job summary

NADAP Inc. is seeking a Bilingual Care Navigator to join our fully remote team serving Medicaid clients in New York.

You will support Care Managers with assessment, care planning, and service coordination for low to medium risk clients, collaborating with medical and behavioral health providers to improve access and outcomes. Requirements include a high school diploma, one year of experience, and fluent Spanish.

Qualifications

• High school diploma required; bachelor’s degree preferred.
• Bilingual Spanish speaker required.
• At least one year of work experience.
• Ability to assess needs and coordinate services.
• Strong communication with clients, Care Managers, and providers.

Responsibilities

• Assist Care Managers with health home care management services through outreach and planning.
• Develop and document daily appointment schedules; inform supervisor of changes and maintain records.
• Facilitate referrals to medical, behavioral health and social services as needed.
• Participate in supervision as scheduled by the supervisor.
• Provide phone support including wellness checks and appointment reminders.
• Support with HRA 2010e applications and provider follow-ups.
• Use EHR/health record systems and NADAP databases to maintain documentation.
• Performs other duties as assigned.
