Job summary

North Bay Regional Center is seeking a Social Services Case Manager in California to manage service delivery for clients, including those with intensive needs. You will oversee Individual Program Plans and coordinate with service providers to access resources and ensure goals are pursued.

Qualifications include a Bachelor’s degree in a related field; a Master’s degree is preferred, and at least 3 years of experience with developmental disabilities. Bilingual Spanish/English is required.

Qualifications

• Bachelor’s Degree in a field related to developmental disabilities (e.g., Social Work, Psychology, Counseling, Special Education, Nursing).
• Master’s Degree preferred.

Responsibilities

• Using person-centered planning to assist clients in accessing resources before using regional center funds to purchase services.
• Monitor vendor services and verify services are delivered per authorizations.
• Evaluate effectiveness of Individual Program Plans and ensure community resources are available.
• Conduct annual reassessments of clients’ developmental status with the interdisciplinary team.
• Conduct client assessments on an occasional basis.
