Job summary

North Bay Regional Center is seeking a Social Services Case Manager to coordinate services for clients with developmental disabilities in Napa, Sonoma, and Solano counties. You will support clients to achieve Individual Program Plan goals, working with interdisciplinary staff and community resources.

The role requires a Bachelor’s in a related field, a Master’s degree preferred, and bilingual Spanish-English abilities.

Qualifications

• Bachelor’s Degree in a field related to developmental disabilities (e.g. Social Work, Psychology, Counseling, Special Education, and/or Nursing).
• Master’s Degree preferred.
• 3 years’ experience with individuals with developmental disabilities.
• Bilingual language ability (Spanish to English) is required.

Responsibilities

• Use person-centered planning to help clients access public and private resources.
• Monitor vendor services and verify services align with service authorizations.
• Evaluate and update Individual Program Plans with interdisciplinary team.
• Conduct annual reassessments and client assessments as needed.
