Qualifications

• Bachelor’s Degree from an accredited school of social welfare or related field; Master’s Degree preferred.

Responsibilities

• Assess individuals and families, develop and coordinate Individual Program Plans.
• Manage a wide range of active cases and coordinate services.
• Prepare reports and ensure records meet federal/state requirements.
• Advocate for clients to obtain entitled services and resources.
• Perform outreach and collaborate with community resources as needed.
