Job summary

North Bay Regional Center in California is seeking a Social Services Case Manager (Service Coordinator) to oversee the delivery of services to clients, ensuring Individual Program Plans are met. You will work with families and the interdisciplinary team to coordinate supports for individuals with developmental disabilities.

The role emphasizes person-centered planning, monitoring service providers, and updating plans.

Qualifications

• Bachelor’s Degree in a field related to developmental disabilities is required.
• Master’s Degree preferred.
• 3 years’ experience with individuals with developmental disabilities.
• Bilingual Spanish to English preferred.

Responsibilities

• Use person-centered planning to assist clients in using public and private resources before using regional center funds to purchase services.
• Monitor vendor services and verify services are delivered per purchase of service authorizations.
• Evaluate and adjust clients’ Individual Program Plans with the interdisciplinary team.
• Conduct occasional client assessments.
