Job summary

North Bay Regional Center in California is seeking a Social Services Case Manager to join our dedicated team. You will coordinate services for NBRC clients, including those with intensive physical or emotional needs, ensuring the Individual Program Plan and its objectives are pursued.

This role emphasizes person-centered planning, collaboration with service coordinators, and connecting clients with public and private resources before using regional center funds.

Qualifications

• Bachelor’s Degree in a field related to developmental disabilities (e.g. Social Work, Psychology, Counseling, Special Education, Nursing).
• Master’s Degree preferred.
• At least 3 years’ experience with individuals with developmental disabilities.
• Bilingual ability (Spanish to English) preferred.

Responsibilities

• Manage service delivery for NBRC clients, including those with intensive needs.
• Use person-centered planning to optimize resource use before utilizing regional center funds.
• Conduct annual reassessments with the interdisciplinary team.
• Perform client assessments as needed.
