Job summary

North Bay Regional Center is seeking a Social Services Case Manager to coordinate services for clients with developmental disabilities. You will manage individualized plans, assess needs, and guide families through available resources, ensuring the best outcomes for clients in Napa and surrounding counties.

Qualifications

Bachelor’s Degree in a field related to developmental disabilities.
Master’s Degree preferred.
3 years’ experience working with individuals with developmental disability.
Bilingual language ability (Spanish to English) is required.

Responsibilities

Use person-centered planning to assist clients in using public and private resources before NBRC funds.
Monitor and evaluate progress towards Individual Program Plans.
Conduct annual reassessments with the interdisciplinary team.
Conduct client assessments on an occasional basis.
