Job summary

North Bay Regional Center is seeking a Social Services Case Manager to coordinate services for clients with developmental disabilities. The role involves managing service delivery to NBRC clients, including those with intensive needs, and ensuring goals in Individual Program Plans are pursued.

The position requires a Bachelor’s Degree in a related field; Master’s Degree preferred, plus 3 years’ experience. Bilingual Spanish/English is preferred.

Qualifications

• Bachelor’s Degree in a field related to developmental disabilities (i.e. Social Work, Psychology, Counseling, Special Education, and/or Nursing).
• Master’s Degree preferred.
• 3 years’ experience working with individuals with developmental disability.

Responsibilities

• Using person-centered planning to assist clients in accessing public and private resources.
• Monitor vendor services and verify services match purchase of service authorizations.
• Evaluate effectiveness of Individual Program Plans and connect clients with community resources.
• Conduct annual reassessments and occasional client assessments with the interdisciplinary team.
