Job summary

North Bay Regional Center is seeking a Social Services Case Manager to coordinate services for individuals with developmental disabilities in Napa, Sonoma, and Solano counties. The position requires a Bachelor's in a relevant field, with a Master’s degree preferred, and at least 3 years of experience.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Counseling, Special Education, Nursing, or related field.
• Master’s Degree preferred for this role.
• 3 years’ experience working with individuals with developmental disability.
• Bilingual language ability (Spanish to English) is required.

Responsibilities

• Use person-centered planning to assist clients in accessing public and private resources before utilizing regional center funds.
• Monitor vendor services and verify services are delivered per POAs.
• Evaluate the effectiveness of clients’ IPPs and connect them with appropriate community resources.
• Conduct annual reassessments of clients’ developmental status with the interdisciplinary team.
• Perform client assessments as needed.
