Job summary

Greater Springfield Senior Services, Inc. is seeking a full-time PCM program role to assess and coordinate tasks, helping clients develop skills to manage PCA services and daily living activities.

Hybrid work may be possible after six months. The position requires English/Spanish fluency, a high school diploma (bachelor's preferred), a valid driver's license, and willingness to travel; PSLF eligibility for nonprofit employees is noted.

Qualifications

• Assesses the ability to independently coordinate tasks within the PCM program.
• Assists consumers in developing skills and resources to manage PCA services and activities of daily living.
• Requires good assessment skills, home visits, knowledge of community services.
• Demonstrates good oral and written communication and accurate data entry.
