Qualifications

Responsibilities

• Assesses ability to independently coordinate tasks for the PCM program.
• Assists consumers in developing skills and resources to manage PCA services.
• Conducts home visits and maintains knowledge of community services.
• Demonstrates good oral and written communication and data entry skills.
