Greater Springfield Senior Services Inc. is seeking a full-time PCM coordinator for Springfield, MA.

The role involves assessing and coordinating tasks for the Personal Care Management program, assisting clients to develop skills for PCA services and daily living activities, and performing home visits with strong communication and data-entry skills. Hybrid schedule may be possible after six months, with travel reimbursement at $0.725 per mile and focus on coordinating services through knowledge

Formación

• Bilingual fluency in English and Spanish required.
• High School Diploma or equivalent required.
• Bachelor's degree preferred.
• Valid driver's license and reliable transportation.
• Must be able to climb multiple flights of stairs.

Full-time 35 hours per week

Hybrid schedule may be possible after six months.

Assesses the ability to independently coordinate tasks associated with the Personal Care Management (PCM) program, assists consumers in developing skills and resources to manage PCA services and activities of daily living. Requires good assessment skills, home visits, knowledge of community services, good oral/written communication, data entry skills.

Travel reimbursement is $0.725/mile

Minimum Qualifications

• Written and Verbal Fluency in English and Spanish is required.
• High School Diploma or equivalent is required.
• Bachelor's degree is preferred.
• Must have valid driver's license and reliable transportation.
• Must be able to climb multiple flights of stairs.

Public Service Loan Forgiveness (PSLF)
