Job summary

Allegheny Intermediate Unit is seeking a community-based service coordinator to assess client needs, develop individualized service plans, and coordinate housing, education, healthcare, employment, and social services to support well-being. The role requires fluency in English and Spanish, strong interpersonal and organizational skills, and experience in case management with diverse populations.

Proficiency with Microsoft Office and case management software is preferred.

Qualifications

• Bachelor's or Associates degree in Social Work, Human Services, Public Health, or related field; equivalent work experience may be considered.
• At least 2 years of experience in service coordination, case management, or similar roles, preferably with diverse and multilingual populations.

Responsibilities

• Assess client needs through interviews and ongoing communication.
• Develop and implement individualized service plans to address the client's needs.
• Coordinate services across housing, education, healthcare, employment, and social services to support client well-being.
• Work with community agencies and healthcare providers to ensure clients receive resources.
• Provide translation and interpretation to assist non-English speakers.
• Advocate for clients within education, social services, healthcare, and legal systems.
• Monitor progress of service plans and maintain client records.
• Collaborate with teams and attend meetings to coordinate services.
