Job summary

IBERO American Action League seeks a Community Habilitation Coordinator to coordinate and monitor services that support individuals with developmental disabilities in achieving independence and community integration.

The role ensures person-centered planning in line with OPWDD regulations and requires collaboration with staff across programs, monthly reporting, and ongoing staff supervision.

Qualifications

• Associate’s degree in Human Services or related field preferred or three years’ OPWDD experience.
• Bilingual English/Spanish required.
• Valid NYS driver’s license and Vehicle Operator compliance.
• Knowledge of OPWDD regulations and person‑centered planning.
• Strong organizational, communication, and leadership skills.

Responsibilities

• Coordinate and monitor services to help individuals develop independence and community integration.
• Maintain relationships with staff from other agencies and IBERO programs.
• Monitor service delivery to ensure goals and outcomes are met.
• Ensure procedures comply with laws, policies, and program rules.
• Meet monthly with supervisor to review issues and plan ahead.
• Attend trainings and update written plans every six months.
• Research and link clients with appropriate services.
• Complete written individualized service plans and reports monthly to the Director.
• Develop individual progress toward goals in the service plan.
• Provide advocacy and support to individuals and families as needed.
• Schedule and supervise Community Habilitation staff.
• Complete satisfaction surveys to gather feedback on services.
