Qualifications

• Bachelor’s degree or higher in related field.
• Experience with infants, toddlers, or families.
• Experience with developmental delays or disabilities.
• Knowledge of Early Intervention, Medicaid-funded programs, or community services.
• Experience with case management or service coordination.
• Strong written and verbal communication skills.
• Strong organizational and time-management abilities.
• Ability to manage multiple cases and documentation requirements.
• Ability to communicate professionally with families, providers, and officials.
• Ability to work independently while following supervision.
• Maintain confidential records.

Responsibilities

• Serve as the primary Service Coordinator for assigned infants, toddlers, and families.
• Explain the Early Intervention Program, service coordination process, family rights, safeguards, and services.
• Communicate with families in their preferred language when qualified to do so.
• Develop family-centered relationships with parents and caregivers.
• Assist families with navigating the Early Intervention system.
• Coordinate communication among families, evaluators, and providers.
• Participate in and coordinate IFSP meetings as required.
• Assist families in identifying priorities and outcomes for their child and family.
• Support transition planning and EI service initiation as appropriate.
• Maintain confidential records and required documentation.
