Job summary

Suvida Healthcare LLC is seeking a Guia to coordinate care for a panel of Medicare-eligible Hispanic seniors. You will collaborate with a multidisciplinary team to meet preventive, chronic, and acute care needs, guiding patients and families toward healthier choices.

The role emphasizes home and community-based visits, social resource navigation, and patient education to improve outcomes. Fluency in English and Spanish is required, with strong communication across settings.

Qualifications

• Community Health Worker certification
• Bachelor's degree (preferred)
• Experience in healthcare settings or related field
• Experience serving senior/geriatric populations
• Bilingual English/Spanish is highly valued

Responsibilities

• Provides comprehensive care coordination to an assigned patient caseload.
• Collaborates with patients, family, caregivers, healthcare providers, and external partners to meet complex social needs.
• Promotes collaborative communication among healthcare team members to ensure integrated care services.
• Intervenes with patients and families regarding emotional, social, and financial consequences of illness.
• Conducts in-person visits to patients' homes, as needed.
• Accesses and mobilizes family/community resources to meet social care needs.
• Documents interventions in the patient medical record across settings (home, clinic, telehealth).
• Onboards patients to the Suvida model and their medical/social care visits.
• Provides patient education on acute and chronic disease management.
• Maintains patient-centered healing relationships with families.
• Assists with scheduling, referrals, and information sharing within the care team.
• Tracks patient enrollment and progression through care programs.
• Performs other duties as assigned by the Guia Manager
