Job summary

Qualifications

• 4-5 years of healthcare experience or relevant experience.
• Experience connecting patients with community resources and governmental agencies to address complex social needs.
• Experience caring for senior/geriatric populations.
• Ability to work independently and collaboratively with a multidisciplinary team.
• Strong organizational and time management skills.
• Knowledge of SNAP, LIS, PAP and prescription assistance programs.
• Excellent oral and written communication skills.
• Proficiency with EMRs and Microsoft Office Suite.
• Experience addressing social determinants of health.

Responsibilities

• Provides comprehensive care coordination to an assigned patient caseload.
• Works with patients, families, caregivers, healthcare providers, and external partners to meet complex social needs.
• Promotes collaborative care and communication among health care team members to ensure integrated services.
• Intervenes with patients and families regarding emotional, social, and financial aspects of illness.
• Conducts in-person home visits as needed.
• Accesses and mobilizes community resources to meet social care needs.
• Documents all interventions in the patient medical record.
• Onboards patients to Suvida model of care.
• Provides patient education on disease management.
• Guides patients and families toward healthier choices.
• Establishes healing relationships with patients and families.
• Communicates with patients across in-person, phone, video, and text.
• Collaborates with Guia Manager and care team.
• Maintains knowledge of Medicare/Medicaid benefits to assist with resource allocation.
• Assists with coordination of care across the continuum.
• Participates in daily operations of the practice and tracks patient enrollment.
