Job summary

Suvida Healthcare is seeking a Guia to coordinate care for Medicare-eligible Hispanic seniors within a multidisciplinary primary care setting. You will engage patients and families, linking them to medical and social services, and support at-home, clinic, and remote visits.

The role emphasizes collaboration with physicians, nurses, and social workers to address social determinants of health and improve outcomes. Fluency in English and Spanish is required.

Qualifications

• 4-5 years of experience working in healthcare setting or relevant experience.
• Experience connecting patients with community resources and governmental agencies addressing complex social needs.
• Experience managing the needs of Senior/Geriatric populations.
• Ability to work independently and develop collaborative relations with physicians, families, patients, interdisciplinary team members, and community agencies.
• Strong organizational and time management skills; ability to prioritize multiple tasks.
• Knowledge of benefit applications such as SNAP, LIS, PAP, and prescription assistance.
• Effective oral and written communication skills.
• Proficiency with EMRs, computers, mobile devices, medical devices, and Microsoft Office Suite.
• Experience utilizing EMRs and social service referral management software.
• Experience assessing social determinants of health.
• Excellent therapeutic communication with patients, families, and caregivers.
• Able to articulate Suvida Healthcare’s mission in relation to patient satisfaction and outcomes.
• Compassionate, kind, and open-minded; able to work across cultures.
• Bilingual/Bicultural (English and Spanish).

Responsibilities

• Provides comprehensive care coordination to an assigned patient caseload.
• Worsk collaboratively with patients, family, caregivers, healthcare providers, and external partners to meet complex social needs.
• Promotes collaboration and communication within care teams to ensure integrated care services.
• Intervenes with patients and families regarding emotional, social, and financial consequences of illness.
• Conducts in-person visits to the patient’s homes and supports in-home visits as needed.
• Accesses and mobilizes family/community resources to meet social care needs.
• Documents interventions in the patient medical record timely and accurately.
• Onboards patients to the Suvida model and their medical/social care visits.
• Provides patient education on acute and chronic disease management.
• Establishes healing relationships with patients and families.
• Communicates with patients in-person, by phone, video, and text.
• Collaborates with Guias, Transitions of Care managers, and Medicaid case managers.
