Job summary

Suvida Healthcare is seeking a Guia to manage a patient panel within a multidisciplinary primary care team. You will help patients meet preventive, chronic, and acute care needs, engaging families and linking them to social resources in-home, clinic, and community.

The role emphasizes building trusting relationships, coordinating care across providers, and guiding patients through Medicare/Medicaid benefits.

Qualifications

• 4-5 years of healthcare experience or relevant experience.
• Experience linking patients to community resources and closed-loop referrals.
• Experience with senior/geriatric populations.
• Ability to work independently and collaboratively with a multidisciplinary team.

Responsibilities

• Provide comprehensive care coordination to an assigned patient caseload.
• Collaborate with patients, families, caregivers, and partners to meet complex social needs.
• Facilitate communication among care team to ensure integrated services.
• Assist with scheduling, referrals, and information sharing across care teams.
• Document interventions in patient records across clinical settings.
