Job summary

Suvida Healthcare seeks a Guia to coordinate care for a panel of Medicare-eligible seniors, focusing on preventive, chronic, and social needs. You will build trust, educate patients, and work with a multidisciplinary team to improve outcomes.

Qualifications

• Community Health Worker certification in required regions.
• Bachelor’s degree preferred; bilingual English/Spanish abilities.
• Experience coordinating care and connecting patients with community resources.
• Proficiency with EMR systems and standard office software.
• Strong communication and collaboration with diverse teams.

Responsibilities

• Provide comprehensive care coordination for assigned patient caseload.
• Collaborate with patients, families, caregivers, providers, and community partners.
• Assist in arranging referrals, appointments, and access to services.
• Document interventions in the patient record and maintain privacy.
• Engage patients in home visits and clinical settings to support care plans.
