Job summary

Suvida Healthcare seeks a Guia to coordinate care for Medicare-eligible Hispanic seniors through a multidisciplinary primary care team. The Guia builds trust with patients and families, helps with social determinants, and arranges services in-home, in-clinic, and remotely to meet preventive, chronic, and acute needs.

You will collaborate with physicians, nurses, case managers, and community partners, using EMRs and referral management tools.

Qualifications

• 4-5 years of experience working in healthcare setting or relevant experience.
• Experience connecting patients with community resources and governmental agencies.
• Experience managing needs of Senior/Geriatric populations.

Responsibilities

• Provides comprehensive care coordination to an assigned patient caseload.
• Works with patients, families, caregivers, healthcare providers, and external partners to meet social needs.
• Promotes collaborative communication among health care team members to ensure integrated care services.
• Intervenes with patients and families regarding emotional, social, and financial consequences of illness.
• Documents all interventions in the medical record and coordinates care across home, clinic, and telehealth.
• Onboards patients to the Suvida model and supports medical/social care visits.
