Job summary

Suvida Healthcare seeks a Senior Guia to provide clinical social work and complex case management for Medicare-eligible Hispanic seniors. You will assess care plans, coordinate resources, and work with a multidisciplinary team to support high-risk patients across home, clinic, and facility settings.

Requires LMSW or equivalent with CHW certification and 3–5 years of social work experience; knowledge of government resources and LIS/PAP/SNAP processes is essential. Equal opportunity employer.

Qualifications

• Experience caring for Senior/Geriatric populations.
• Ability to assess social determinants of health and coordinate resources.
• Working knowledge of ICD-10 coding.
• Proficient in EMRs and Microsoft Office.
• Strong therapeutic communication with patients and families.
• Experience with pre-acute and post-acute care settings.
• Connecting patients with community and government resources.
• Independent worker with collaborative skills.

Responsibilities

• Provides comprehensive care coordination to an assigned caseload.
• Collaborates with patients, families, caregivers, and providers for complex needs.
• Develops and carries out treatment plans using clinical social work methods.
• Addresses emotional, social, and financial consequences of illness.
• Assesses and mobilizes family/community resources for social care needs.
• Intervenes in elder abuse/neglect, domestic violence, guardianship, and related areas.
• Assists with advance directives and end-of-life planning.
• Coordinates transitions of care with post-acute providers.
• Maintains documentation in the medical record across settings.
• Advocates for patient access to Medicare/Medicaid and benefits.
