Job summary

Suvida Healthcare is seeking a Guia to coordinate care for Medicare-eligible Hispanic seniors, working with a multidisciplinary team to meet preventive, chronic, and social needs.

The Guia builds trusting relationships with patients and families, assists with social determinants of health, and supports in-home, clinic, and remote care delivery. Community health worker background and bilingual skills are valued.

Qualifications

• 4-5 years of experience in healthcare setting or relevant experience.
• Experience connecting patients to community resources and addressing social needs.

Responsibilities

• Provides comprehensive care coordination to an assigned patient caseload.
• Collaborates with patients, families, and community partners to meet social needs.
• Documents interventions in the patient medical record and coordinates care across the continuum.
• Onboards patients to the Suvida model and supports their medical/social visits.
• Engages patients to promote healthy lifestyle choices and self-efficacy.
