Job summary

Suvida Healthcare is seeking a Guia to coordinate care for a panel of Medicare-eligible Hispanic seniors. The Guia collaborates with physicians, nurses, and social services to meet preventive, chronic, and social needs. You will engage families and patients in home visits, telehealth, and community resources.

The role emphasizes culturally competent care, social determinants of health navigation, and closed-loop referrals to optimize outcomes in a community-focused clinic setting.

Qualifications

• 4-5 years of experience working in healthcare setting or relevant experience
• Expertise connecting patients and ensuring closed loop referral with community resources and governmental agencies
• Experience managing the needs of Senior/Geriatric populations
• Ability to work independently and with physicians, families, patients, and care teams
• Strong organizational and time-management skills; ability to prioritize tasks
• Proficiency with EMRs, computers, mobile devices, and Microsoft Office

Responsibilities

• Provides comprehensive care coordination to an assigned patient caseload
• Collaborates with patients, family, caregivers, healthcare providers, and external partners to meet social needs
• Promotes a collaborative process and communication between all health care team members and families
• Intervenes with patients and families regarding emotional, social, and financial consequences of illness
• Conducts in-person visits to the patient’s homes as needed
• Accesses and mobilizes family/community resources to meet social care needs
• Documents all interventions in the patient medical record including in home and remote encounters
• Onboards patients to the Suvida model and care visits
• Provides patient education on disease management and healthy behaviors
• Maintains knowledge of Medicare/Medicaid benefits to assist patients
• On supports coordination of care across continuum (scheduling, referrals, transfer of information)
• Participates in daily operations of the practice and tracks patient enrollment
