Job summary

Suvida Healthcare is seeking a Guia to support a panel of Medicare-eligible Hispanic seniors through coordinated care across the primary care team. The Guia helps patients meet preventive, chronic, and social needs while fostering trust and self-management.

The ideal candidate holds a Community Health Worker certification, 4–5 years in healthcare, and is bilingual in English and Spanish. Responsibilities include home visits, referrals, documentation, and education, with a strong emphasis on

Qualifications

• 4–5 years of experience in healthcare settings.
• Community Health Worker certification required.
• Bilingual in English and Spanish.
• Strong organizational and time-management skills.
• Experience coordinating care for seniors.
• Knowledge of SNAP, LIS, PAP and prescription assistance is a plus.

Responsibilities

• Provide comprehensive care coordination for an assigned patient caseload.
• Collaborate with a multidisciplinary team to meet social needs.
• Assist with scheduling, referrals, and sharing information.
• Document all interventions in the patient medical record.
• Onboard patients to the Suvida care model.
• Educate patients on chronic disease management.
• Support care in-home, in-clinic, and remotely.
