Job summary

SCAN Health Plan in Los Angeles, CA, is recruiting a full-time RN for care management to support frail seniors in aging in place through telephonic care coordination, education, and coaching. You will develop and implement plans of care for Medi-Cal/Medicare members with complex needs, aiming to prevent hospitalizations and ER visits.

Role requires CA RN license, 3+ years in managed care or similar settings, and strong communication skills; bilingual English/Spanish preferred.

Qualifications

• ASN required.
• RN license required (CA).
• BSN preferred; graduate/advanced degree preferred.
• Bilingual English/Spanish preferred; proficiency testing may apply.
• 3+ years in managed care, healthcare, or case management.
• 2+ years working with seniors and remote work preferred.

Responsibilities

• Perform initial and annual telephonic assessments for Medi-Cal/Medicare dually eligible members.
• Perform CICM activities for targeted populations.
• Ensure care plans reflect evidence-based guidelines and best practices.
• Collaborate in interdisciplinary planning and case conferences.
• Provide education, coaching, and disease management for chronic conditions.
