Medix is seeking Spanish-speaking Registered Nurses to perform in-home and community assessments for members in managed long-term care programs. The role uses the Uniform Assessment System (UAS) to evaluate health status, care needs, and eligibility, with emphasis on independent work and direct member interaction.

No prior UAS experience is required; candidates with strong clinical skills and home health experience are encouraged to apply.

Qualifications

• Active NY RN license required.
• Fluent in Spanish and English; interview to assess proficiency.
• Willingness to travel across the Bronx and Manhattan.
• Strong computer skills and comfort with multiple software platforms.
• Proficiency with Microsoft Word, Excel, and Outlook.

Responsabilités

• Conduct in-home and community-based assessments using the Uniform Assessment System (UAS).
• Evaluate members' health status, care needs, and home environment.
• Coordinate scheduling and confirm appointments the evening before.
• Complete documentation in the UAS system within 24 hours of assessment.
