Job summary

FlexStaff is seeking a Registered Nurse with UAS assessment experience to support an external client delivering home healthcare in the New York metropolitan area, primarily serving Brooklyn and Lower Manhattan. This is a full-time, on-site role in Bronx, NY, with a Monday–Friday schedule and 37.5 hours per week.

You will conduct in-home assessments, complete UAS-NY, review medications, and ensure accurate EMR documentation within 48 hours.

Qualifications

• Current active and unrestricted NY RN license is required.
• Minimum of two years of experience in long-term home health care (LHCSA).
• Experience with UAS-NY assessments and in-home visits.
• Willingness to travel across boroughs and work some weekends.

Responsibilities

• Perform in-home UAS-NY assessments to determine eligibility.
• Document assessments and care plans in EMR within 48 hours.
• Collaborate with intake, enrollment, and care teams.
• Educate participants on plan benefits and care model.
• Stay current with CMS/PACE guidelines and organizational policies.
