Job summary

4 Corner Resources is seeking bilingual (Spanish) Field Assessment RNs to conduct in-home member assessments across the Bronx and Manhattan. These are 6-month contract roles with potential to convert to permanent. Monday–Friday, 9:00am–5:30pm, field-based visits are 100% in members' homes.

RNs must be licensed in NY, fluent in Spanish, and reside within a 45-minute commute of the Bronx or Manhattan. Mileage and MetroCard costs are reimbursed.

Qualifications

• Active New York State RN license.
• Fluent spoken Spanish.
• Must live within a 45-minute commute of the Bronx or Manhattan.
• Comfortable with Outlook, Teams, Excel, and basic computer navigation.

Responsibilities

• Conduct health assessments in members' homes in Spanish.
• Use UAS for assessments for prospective and current members.
• Determine enrollment and ongoing eligibility using clinical judgment.
• Perform assessments in skilled nursing facilities as needed.
• Provide feedback to home care agencies and the CMT.
• Update Care Plans or assist with nursing home admissions.
• Deliver community-based training on frail population care.
• Explain enrollment in managed long-term care plans to members.

4 Corner Resources is hiring multiple Bilingual (Spanish) Field Assessment RNs to conduct in-home member assessments across the Bronx and Manhattan. These are 6-month contract roles with potential to convert to permanent. If you’re a New York RN who speaks Spanish fluently and wants a Monday–Friday daytime schedule out in the community this one is for you!

Job Details:

• Duration: 6-month contract with potential for conversion to permanent employment.
• Schedule: Monday–Friday, 9:00am–5:30pm. No weekends required.
• 100% field-based, with visits in members’ homes, primarily in the Bronx and also in Manhattan.
• Mileage and MetroCard costs reimbursed

Duties and Responsibilities:

• Conduct visual (non-hands-on) health assessments in members’ homes, communicating directly with members in Spanish without an interpreter.
• Conduct home assessments using the Uniform Assessment System (UAS) for both prospective and current members.
• Utilize clinical expertise to determine enrollment and ongoing eligibility.
• Conduct assessments in various settings, including skilled nursing facilities, as necessary.
• Provide feedback to home care agencies and Collaborative Management Team (CMT) regarding care issues and aide performance.
• Review previous assessments and medical notes to update Care Plans or assist with nursing home admissions.
• Deliver community-based training on frail population care, including self-care techniques and prevention strategies.
• Ensure clear understanding and agreement from members and their representatives regarding enrollment in managed long-term care plans.

Qualification Requirements:

• Active New York State RN license.
• Fluent spoken Spanish
• Must live within a 45-minute commute of the Bronx or Manhattan.
• Comfortable with Outlook, Teams, Excel, and basic computer navigation.

Preferred Qualifications:

• Home care experience.
• UAS-NY and/or OASIS assessment experience.
• Utilization management or telephonic care management experience.

Schedule:

• Monday–Friday, 9:00am–5:30pm

Pay Rate:

• $56/HR-64/HR
