Job summary

X-Treme Care, a leading home care agency in New York, is seeking a UAS Assessment Nurse to evaluate patients across five boroughs, Long Island, and Westchester County. The role requires NY RN licensure, UAS-NY proficiency, and experience in home care or LTC settings.

The candidate will assess patients, document findings, and communicate clearly with care teams. This is a fee-for-service position with per-case pay and bilingual English/Spanish abilities preferred.

Qualifications

• RN license in New York in good standing.
• Knowledge of UAS-NY Assessments and proficiency with UAS-NY.
• Experience as a NY-licensed RN in home care or LTC/MLTCP setting.

X-Treme Care is a leading home care agency in New York that has been operational for almost 20 years. We are looking for fee for service UAS Assessment nurses to assess patients in the five boroughs, Long Island and Westchester County.

Job Description

Qualifications:

• New York State Registered Nursing (RN) license in good standing.
• Proven knowledge of UAS-NY Assessments and proficiency in use of UAS-NY.
• Experience as a NY licensed RN working in a home care environment or in an acute, sub-acute, or long term care (LTC) setting or managed long-term care (MLTCP).

Skills Required:

• Solid assessment and documentation skills.
• Ability to effectively communicate verbally and in written formats at all levels.
• Ability to use computerized systems such as Microsoft Outlook, Microsoft Word
• Bilingual English/Spanish

Requirements:

Pay Range: $140 -$ 180 per case
