Job summary

X-Treme Care is seeking a UAS Assessment Nurse to perform patient assessments across New York City's five boroughs, Long Island, and Westchester County. This fee-for-service role requires RN licensure and expertise in UAS-NY assessments, with strong documentation and communication skills.

Responsibilities include accurate data collection, timely reporting, and using Outlook/Word/Excel for case management. Bilingual English/Spanish is a plus; travel within service areas is expected.

Qualifications

• New York State RN license in good standing.

• Experience as a RN in home care or LTC/MLTCP settings.

X-Treme Care is a leading home care agency in New York that has been operational for almost 20 years. We are looking for fee for service UAS Assessment nurses to assess patients in the five boroughs, Long Island and Westchester County.

Qualifications:

• New York State Registered Nursing (RN) license in good standing.
• Proven knowledge of UAS-NY Assessments and proficiency in use of UAS-NY.
• Experience as a NY licensed RN working in a home care environment or in an acute, sub-acute, or long term care (LTC) setting or managed long-term care (MLTCP).

Skills Required:

• Solid assessment and documentation skills.
• Ability to effectively communicate verbally and in written formats at all levels.
• Ability to use computerized systems such as Microsoft Outlook, Microsoft Word
• Bilingual English/Spanish
