Location: Bronx and Manhattan, NY | Field-Based/Hybrid

Employment Type: 6-Month Contract

Schedule: Monday–Friday, 8:30 AM–5:00 PM (40 hours per week)

Position Overview

We are seeking Spanish-speaking Registered Nurses (RNs) to conduct in-home and community-based assessments for members enrolled in or seeking enrollment in managed long-term care programs.

The Assessment Nurse will use the Uniform Assessment System (UAS) to evaluate members' health status, care needs, home environment, and eligibility for services. This role is ideal for nurses who enjoy working independently, interacting directly with members, and helping older adults and individuals with complex care needs access appropriate services.

No prior UAS experience is required! Candidates with strong clinical skills, computer proficiency, and experience in home care, community health, case management, or other patient-facing healthcare roles are encouraged to apply.

Important: This is an assessment-focused nursing role. Nurses will not be responsible for physically handling members or administering medications.

Required Qualifications

• Active, unrestricted New York State RN license.
• Active New York State driver's license or valid New York State ID.
• Fluency in Spanish and English; Spanish proficiency will be evaluated during the interview.
• Willingness and ability to travel throughout the Bronx and Manhattan.
• Strong computer skills and comfort navigating multiple software platforms.
• Basic proficiency with Microsoft Office, including Word, Excel, and Outlook.
• Ability to manage documentation, organize electronic files, and meet strict deadlines.
• Strong clinical judgment, communication, time management, and problem-solving skills.
• Ability to work independently and manage a field-based schedule.

Schedule and Workload

• Monday–Friday, 8:30 AM–5:00 PM.
• Approximately 12 assessments per week.
• Typical appointment times are 9:00 AM, 12:00 PM, and 3:00 PM.
• The schedule generally includes two assessments on three days and three assessments on two days each week.
• Appointments will be coordinated by a scheduler, with nurses responsible for confirming appointments the evening before.
• Documentation must be completed in the UAS system within 24 hours of the assessment.
• Nurses may complete documentation in the member's home, in the community, or at home.

Work Location and Travel

This is a field-based position primarily covering the Bronx and Manhattan, with occasional travel to other boroughs as needed.

Candidates ideally reside in the Bronx or Manhattan. Candidates living in New Jersey near the George Washington Bridge may also be considered if they can reasonably commute to the service area.

Candidates must be comfortable navigating the boroughs independently. A personal vehicle is not required if the candidate is confident using public transportation.

Transportation Reimbursement

• Mileage: $0.70 per mile for travel between members' homes. Travel to the first appointment and from the final appointment is excluded.
• Public transportation: An OMNY card valued at $130 is provided.
• Parking expenses are not reimbursed.

Training

The role includes approximately six weeks of training, consisting of:

• Remote training sessions.
• Several onsite training days at the Church Street, New York, NY 10007 office.
• Shadowing opportunities and hands‑on guidance from a preceptor.

Candidates without prior UAS experience are welcome to apply and will receive training.

• Initial six‑month contract to cover staff leaves of absence.
• This is not initially a contract‑to‑hire position.
• Extensions may be possible, and strong performers may be considered for permanent opportunities in the future.
