The Assessment Nurse position with Medix™ involves field-based home assessments for members and prospective members across downstate New York, with documentation to determine enrollment and service needs. You will support the Care Management Team to ensure efficient operations of long-term care plans.

The role focuses on geriatrics/home care contexts, requires travel across the Bronx and Manhattan, and includes six weeks of training with on-site and remote components.

Qualifications

• Must have NY driver’s license or NY state ID.
• Ability to travel around downstate New York up to and including the 5 boroughs, Long Island, Rockland, and Westchester.
• Proficiency with Microsoft Word, Excel, and Outlook.
• Experience navigating multiple technology platforms and documenting assessments.

Responsabilités

• Conducts home assessments (and virtual assessments, as needed) for prospective members and re-assessments of current members using the Uniform Assessment System (UAS); determine enrollment and continued eligibility.
• Navigates multiple technology/digital platforms such as UAS software, electronic medical records, TruCare, etc., to conduct and document comprehensive assessments.
• Completes assessment of service needs per CMT and member's family/caregiver’s request to determine service needs or any necessary adjustments.
• Reviews previous nursing assessments/medical notes to determine necessary adjustments/updates in Care Plan and/or to assist with nursing home admission.
• Conducts in-person Care Management visits in support of person-centered service planning.
