Responsibilities

• Coordinate monthly visits for patients at residential care homes.
• Answers phones, screen and triage calls, and direct calls appropriately.
• Schedule medical necessity visits including follow ups and vaccines.
• Contacts facilities to offer follow-up for missed appointments.
• Update residential care master schedule daily and maintain facility list.
• Develop relationships with providers and facilities via telephone.
• Provide customer service to 550+ facilities and assigned providers.
• Attend required meetings and trainings.
