Job summary

HumanGood is seeking a dedicated Resident Services Coordinator in Conshohocken to organize and evaluate Social Services for residents, ensuring links to supportive services and independent living in the community. The role follows a full-time schedule, Monday through Friday, with a 9:00 AM–5:00 PM workday.

You will assess needs, promote positive living experiences, and maintain confidentiality under HIPAA while collaborating with the team to support residents and their families.

Qualifications

• 1+ years of experience in case management required.
• 2+ years of social services experience, preferably in a senior care setting
• Bachelor’s Degree required in social work or related program such as gerontology, psychology, or recreation therapy
• Bilingual - English/Mandarin
• Social Services certification preferred

Responsibilities

• Assess resident needs based on admission social services history, resident’s health, psychological and social needs
• Promote a positive living experience for residents
• Ensure confidentiality by adhering to HIPAA & all other privacy standards
• Promote a positive work environment by collaborating with community team members
