Enlyte in Houston seeks a Field Case Manager to coordinate care for injured workers. You will partner with physicians, employers, and clients to design and monitor treatment plans, aiming for safe, cost‑effective recovery and timely return to work.

This role blends clinical expertise with case management to support outcomes. We value bilingual English/Spanish, strong communication, independence, and reliable travel within the geography.

Qualifications

• Education: Associates Degree or Bachelor’s Degree in Nursing or related field.
• Experience: 2+ years clinical practice preferred; workers’ compensation experience preferred.
• Certifications: Active RN license required; URAC case management certifications preferred (CCM, CDMS, CRC, CRRN, COHN, etc.)

Responsabilités

• Demonstrate knowledge of case management standards of practice.
• Advise on treatment plans based on injuries and job duties.
• Interview disabled persons to assess recovery and whether injuries are occupational or non-occupational.
• Collaborate with treating physicians/providers to tailor treatment plans.
• Work with employers/physicians to modify job duties for early return to work.
• Evaluate and modify case goals based on patient progress.
• Independently manage workload and priorities.
• Attend appointments and prepare status updates for customers.
