Qualifications

• Active, unrestricted RN licensure in the state of residence.
• BLS or CPR certification.
• Fluent written and spoken English.
• US work authorization.
• EMR experience.
• Ability to maintain patient confidentiality and process information confidentially.
• Ability to assess patients without face-to-face interaction.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage.
• Execute protocol-driven chronic condition management.
• Perform nursing procedures including wound care and vaccinations.
• Educate patients and caregivers on chronic conditions and preventive care.
• Review medications within nurse-led visits for safety and adherence.
• Conduct onsite testing (point-of-care labs, EKGs) and use data to close gaps in screenings.
