Job summary

Health Plus Management is seeking a Patient Care Coordinator to act as the liaison between physicians and patients, greeting patients and delivering exceptional customer service throughout visits. You will educate patients on their plans and respond with compassion.

Requirements include 1+ year medical experience (preferred), knowledge of medical terminology and EMR systems, and strong English/Spanish communication. This full-time role offers a pay rate of $19–$21 per hour.

Qualifications

• 1+ years of medical experience (preferred)
• Knowledge of medical terminology & procedures; able to converse with patients
• Knowledge of EMR systems
• Strong computer skills; Microsoft Word/Excel proficiency

Responsibilities

• Provide outstanding customer service to every patient
• Coordinate patient visits to ensure progress of treatment plans
• Educate patients on treatment plans and practice policies
• Schedule treatment plans with patients
• Track referrals and treatment plans
• Assist patients in setting goals and addressing problems
• Assist with completion of paperwork
• Respond to patient inquiries with compassion and timeliness
• Process insurance information; verify eligibility; liaise with Billing
• Ensure confidentiality of patient information (HIPAA)
• Maintain patient demographics and insurance information in systems
• Provide advocacy, information and referrals to patients
• Act as back-up to team members
• Other duties as assigned
