Job summary

Institute on Aging is seeking a compassionate outreach specialist to enroll new clients into enhanced care management, starting with telephonic contact and home visit assessments as needed. You will educate communities about services and ensure proper consent and enrollment.

The role requires bilingual English/Spanish communication, strong organization, and the ability to document interactions accurately in EHR systems, serving a diverse client base.

Qualifications

• One year of experience with disabled-adult or underserved populations.
• Lived experience in the community served is valuable.
• Detail-oriented with strong organizational skills.
• Clear and effective communication abilities.

Responsibilities

• Initiate telephonic contact with newly eligible enhanced care management clients after health plan authorization.
• Complete face-to-face visits to enroll clients if they consent to home visits.
• Provide education about program services to clients and community organizations.
• Obtain consent to receive services and enroll new clients.
• Maintain documentation of outreach activity in the program's electronic health record.
