Job summary

Integrated Practice Management LLC in California is seeking an Outreach Specialist to support the Enhanced Care Management program by conducting outreach, enrolling members, and coordinating care across the interdisciplinary team.

You will travel up to 20% and engage with members in clinics, homes, and community settings, ensuring timely enrollments, follow-ups, documentation, and resource linkage to improve health outcomes.

Qualifications

• High school diploma or equivalent.
• Spanish proficiency required.
• Experience in patient outreach preferred.
• Strong interpersonal and communication skills.
• Ability to work independently within a multidisciplinary team.
• Proficiency with Microsoft Office and EHR systems preferred.
• Strong organizational skills and attention to detail.

Responsibilities

• Conduct outreach to eligible members and facilitate enrollment into ECM and programs.
• Complete enrollments and program documentation per guidelines.
• Maintain ongoing communication to support engagement and continuity of care.
• Provide care coordination: scheduling, referrals, coordinating services, follow-up.
• Identify barriers to care and link to resources.
• Support Lead Care Managers with follow-up and documentation.
• Collaborate with providers, community partners, and internal teams.
• Conduct home and community visits as needed to engage members.
• Monitor progress and communicate concerns to care team.
• Maintain accurate documentation of outreach and activities.
• Ensure compliance with ECM requirements and privacy standards.
• Perform other duties in support of ECM programs.
