Legacy Home Health Agency, Inc. is seeking a Community Outreach Specialist to grow client census and satisfaction while delivering compassionate, in-home or community-based care.

The role involves field visits across the community, building relationships with doctors, centers, and facilities, and expanding outreach programs. The position emphasizes bilingual English/Spanish communication, reliable transportation, and the ability to engage Medicaid-eligible clients and senior care facilities.

Qualifications

• Three years of customer service experience preferred.
• One year in marketing or community outreach preferred.
• Bilingual in English and Spanish preferred.

Responsibilities

• Identify prospective patients/clients for agency services.
• Interact with patients/clients, family members, caregivers and other members of the community to assess customer satisfaction with agency services.
• Develop and implement programs to expand the agency's community outreach.
• Participate in community awareness programs, such as events and health fairs and/or clinics as appropriate
• Build relationships on behalf of the agency with local doctors offices, community centers, senior living facilities or programs.
