Oak Street Health is seeking a Nurse Practitioner who provides effective, equity-focused primary care to adults on Medicare. You will work on a collaborative care team, see patients independently, and perform wellness visits along with comprehensive documentation.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary state NP license; Active DEA license.
• US Work Authorization; Bilingual in Spanish.

Responsibilities

• Provision of effective and equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Perform annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers, specialists, testing facilities, and agencies.
• Lead population health efforts with the care team to improve screening rates and outcomes.
• Educate patients about health conditions, care plans, and treatments.
