Job summary

The Hispanic Alliance for Career Enhancement is seeking a Nurse Practitioner to provide effective and equitable primary care to Medicare adults. The role requires proficiency in both English and Spanish, as well as certifications in relevant nursing specialties.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Geronotology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner.
• Active, non-probationary state Nurse Practitioner license and active DEA license.
• US Work Authorization.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Ensure care coordination with other providers.
• Lead population health initiatives with the care team.
• Educate patients on health conditions and care plans.

Skills

Bilingual proficiency in Spanish

Experience in primary care

Knowledge of HCC documentation

Education
